Sports Physicals
    Friend of the Crusaders and fellow parishioner, Dr. Eckberg, will hold a Sports Physicals Day for all school age athletes and siblings.  Physicals will be valid for all sports, (and most camps), for one year.

    Appointments are necessary and will fill up quickly, so please call Sheri Mazzocchi @ 630-466-9515 or email at imsharris@mchsi.com to reserve your appointment time today.  If you leave a message, please let me know what time you are requesting and I will give you a confirmation call in return.  Thank you!

 $25 per child / $40 per family

Please note: Sports physicals do not include administering any vaccines or validating vaccine records. You may, however, obtain a vaccine record from your regular physician and attach it to the physical form if it is required.
