CRECHE Registration Forms

Annual dues for CRECHE are $24. Any returning members who have not paid (or mailed) by August 13, 2011, will be required to pay a $6 late fee. Payment and mailing information is on the CRECHE registration form.

You will be notified and given access to the CRECHE website and Yahoo! group as soon as your CRECHE registration is complete.  If you have any questions, please click here to contact us.

To register for CRECHE, you must fill out 3 forms: (1) the CRECHE Release and Waiver Form, (2) the CRECHE Registration Form, as well as (3) the CRECHE Code of Conduct Form. All forms should be brought with you to registration or mailed to the CRECHE registrar along with a check made payable to CRECHE to the address on the forms.

CRECHE Reimbursement Request

If you need to be reimbursed by CRECHE for funds used at a CRECHE sponsored event, please click here to download the Reimbursement Request form. Submit completed forms to the treasurer.
