TORCH / CRECHE Registration Forms

You will need Adobe Acrobat in order to view the pdf files below. Data can be entered directly into the T.O.R.C.H. forms. Unfortunately, the C.R.E.C.H.E. pdf form does not allow for direct data entry.

However, there is a Word document that does allow for direct data entry.

To register for CRECHE, you must fill out 4 forms: (1) the TORCH Registration Form, (2) the TORCH Release and Waiver Form, (3) the CRECHE Registration Form, as well as (4) the CRECHE Code of Conduct Form.
